What does “diabetes management” at a GP clinic involve?

A planned review of your health history, medicines, blood pressure/weight, blood tests (e.g., HbA1c), foot checks, and referrals (dietitian, podiatry, eye checks) with follow-ups to keep you on track.

Most people are reviewed every 3 months; if your levels are stable, every 6 months may be appropriate—your GP will individualise this.

Targets are individualised. A common goal for many adults is ≤7% (53 mmol/mol), adjusted for your age, risks and preferences.

From 1 July 2025, GP Management Plans/Team Care Arrangements were replaced by a single GP Chronic Condition Management Plan (GPCCMP). You can still be referred for up to 5 individual allied health visits per calendar year (10 for Aboriginal and Torres Strait Islander people) and, if you have type 2 diabetes and are suitable, up to 8 group services each year.

The NDSS supports people living with diabetes through education and subsidised products (e.g., strips; some CGM/Flash devices for eligible groups). Speak with your GP/diabetes educator to register and access benefits.

As at 2025, all people with type 1 diabetes are eligible to apply for subsidised CGM via the NDSS. Subsidised access is not generally available for type 2 diabetes at this time.

Your GP will plan an “annual cycle of care”, typically including HbA1c, kidney tests (eGFR/urine ACR), blood pressure, lipids, foot assessment, and regular eye screening. Frequency depends on your results and risk.

Influenza vaccination every year is recommended, with funded access for medical-risk groups; pneumococcal vaccination is also recommended for adults with risk conditions such as diabetes (schedule depends on age/risk—your GP will advise).

Yes—history reviews and care-plan updates can often be done by video/phone. You’ll still need in-person visits for physical checks, blood tests and foot exams.

Your blood glucose log/device/app, medication list, any recent pathology or specialist letters, and questions you want to discuss. (If NDSS forms are needed, bring those too.)

Book a prompt review. Your GP may adjust medicines, diet/exercise plans, or refer you to a credentialled diabetes educator. Seek urgent care if you have severe hypoglycaemia, chest pain, shortness of breath, or signs of dehydration/ketosis.

Medicare supports GP care-plan services under the 2025 GPCCMP items; gap fees vary by clinic. Ask reception when booking.
