What is a GP health assessment?

It’s a structured check of your health, lifestyle and risks. It may include questions about your medical history plus checks like weight and blood pressure, to help identify issues early and plan next steps. Common groups include: people aged 75+ (every 12 months), residents of aged care facilities (every 12 months), Aboriginal and Torres Strait Islander people (annual), people aged 45–49 at risk of chronic disease (once only), refugees/humanitarian entrants (once only), people with intellectual disability (annual), and 40–49 with high diabetes risk (every 3 years).

Frequency depends on the assessment type (for example, 75+ is annual; 45–49 at risk is once only; diabetes risk evaluation is every 3 years). Your GP will advise what applies to you.

Your GP/practice nurse will discuss your health and medications and may record vitals (e.g., blood pressure, weight). Taking a medical, social, and family history; If needed, they’ll arrange tests, screening or referrals and create an action plan.

Allow around 20–30 minutes, depending on your needs and whether follow-up tests are required.

If appropriate, your GP may recommend a Chronic Condition Management plan, referrals to allied health, screening, or vaccinations, and set review dates.
